1. Member’s Acceptance of Rules and Policies
Upon acceptance of this application, Member agrees to terms and conditions of this Agreement. Member has reviewed membership rights and privileges and agrees to abide by all rules and policies of the New Ulm Rec Center (subject to change without notice). Member agrees to use this facility at their own risk and to adhere to any applicable Executive Order or similar orders that apply to City Property or the member’s activities, including but not limited physical distance, face coverings and applicable gathering limits.
2. Administrative Fees - Member is required to pay admin fee to set up monthly ACH Payments and shall be included in the monthly fee.
3. Initial Annual Membership
In order to be eligible for household membership, each person must be living full time at the same residence. Staff reserves the right to verify residency via drivers license or some other identification.
4. All debt owed to the City of New Ulm must be paid in full prior to extending additional credit through automatic payments.
5. Rec Center Dues/Building Closure
6. Insurance Reimbursement - Qualified Member is responsible for meeting requirements of insurance reimbursement program, including recording visits.
7. Memberships are non-refundable - In the event the customer decides to discontinue membership, they are still responsible for monthly installment payments until membership is paid in full, unless other arrangements are made, due to extenuating circumstance.
8. Involuntary Termination - City of New Ulm reserves the right to terminate membership at any time, for member’s failure to comply with rules/regulations adopted or for improper conduct.
9. Medical Examination - Members are strongly encouraged to consult and follow their physician’s recommendations regarding all forms of exercise.
10. Proper Attire and Equipment is required; shirts and shoes are required in all public areas. Dry, clean athletic footwear is required on courts, track or in fitness centers.
11. Lockers - no personal belongings shall be left in lockers overnight; anything left will be removed and disposed.
12. Zero Tolerance Negative Conduct Policy is strictly enforced and staff is sole judge of what constitutes negative conduct. If caught, member may be required to leave building and grounds, either temporarily or permanently with no refund. If necessary New Ulm Police may be asked to enforce.
